CSMP 2015-16 ACF


Release Date (w/ CMP Cover Letter): Wednesday January 14, 2015

Eligibility: Currently-funded CMP sites

Submission Deadlines   

Friday, February 20, 2015 at 4:00pm: Sites that receive NCLB funds will be required to submit NCLB budget and budget narrative information to CMP Statewide Office.

Thursday, March 12, 2015 at 4:00pm: All sites required to submit completed State and NCLB application packet (hard and electronic copies) to CMP Statewide Office.

Proposal Submission

Submit one complete proposal for each funding source.  CSMP sites receiving both State and NCLB 10 funds must submit two (2) separate proposals.

Format Requirements

12 pt font;1.5 line spacing;1 inch margins

Name of the CSMP Site and page numbers listed on the bottom of all pages

Part II: Progress Report 2014-15

Part III: Scope of Work 2015-16

Part IV: Budget and Budget Narrative for 2015-16

Proposal documents can be uploaded as:

OPTION 1: Complete proposal w/ scanned signature page in one single PDF + Excel spreadsheet of budget. (preferred)

OPTION 2: Scanned signed signature page in PDF + narrative portions of the proposal in PDF or Word + budget pages in Excel + budget pages in PDF



Hard Copy Submissions

Mail one complete hard copy of each signature page with original signatures to the address below.  (This is the only hard copy requirement.  All other components to be submitted electronically)
